MHI-SLP

Welcome to MHI-Supported Living Program (MHI-SLP)! We provide supports and services to adults qualifying for services through the Indiana Medicaid Home and Community-Based Services Waiver Program.  These individuals will have been diagnosed with a developmental disability prior to the age of 22, or those diagnosed specifically with Autism.  These services include:  Residential Habilitation, Individual Community Habilitation, Community Transportation, Healthcare Coordination and Community Transition Supports.  Please click the link below to visit the Division of Disabilities and Rehabilitative Services website, http://www.in.gov/fssa, for specifics on qualifications to receive a Medicaid Waiver.
